Registration
for workshops
Registration fee for the ISSAM/ WHO Workshop is RM150 (USD 40), which
includes lunch. As places are limited, please register early to avoid
disappointment. (Registration for the GP workshop is free)
Payment
Payment can be made by bank draft or credit card in Ringgit Malaysia/USD,
and submitted together with the registration form. Bank Drafts must
be made payable to the 'Malaysian Society of Andrology'.
Registration
will be confirmed by mail or email upon receipt of payment and will
include an official acknowledgement notice. This document must be presented
during on-site registration.

Entitlements
Nametags must be worn at all times to ensure admission to scientific
sessions, trade exhibition, conference tours, and social events.
Cancellations
and Refunds
Cancellations must be made in writing to the Conference Secretariat
by 10 February 2001. There will be a 30% administrative fee for all
cancellations (to be settled after the conference) and NO
REFUNDS WILL BE PROVIDED AFTER 10 FEBRUARY 2001.

Points to Note for all Delegates

Each
Full Delegate must complete a separate registration form. (photocopies
are acceptable) |
| 2.

To
avoid delay in confirmation of registration, registrants are advised
to remit the correct sum of fees in one payment for all requirements.
Early registration is encouraged. |

Please
notify the Conference Secretariat of any change in mailing address
and any other contact details after registration. |
| 4.
|
Abstracts
for presentations will be considered only if accompanied by registration
form and full payment of fees. All accepted papers will be published
in the international journal, The Aging Male. |

You
are advised to register and collect your materials as soon as possible
after your arrival. |

All
fees are payable in Ringgit Malaysia/USD and the exchange rate quoted
(USD$ 1.00 ~ RM 3.80) is correct at the time of printing. All USD
rates are approximate. The organisers will bear no responsibility
for any possible exchange rate fluctuations after the time of printing. |
